  • With eight years of healthcare experience, including five years serving as an Administrator in Skilled Nursing Facilities, Logan brings a strong background in healthcare leadership and resident-centered care. She earned a Master of Arts in Gerontology with an emphasis in Healthcare Administration and Public Health and became a Licensed Nursing Home Administrator in 2018.

    Logan is passionate about building lasting relationships with residents and their families, often forming connections that span many years. She especially values supporting skilled nursing patients as they work toward regaining strength, independence, and achieving their personal goals through compassionate, quality care. Her leadership philosophy is grounded in integrity, support for staff, and a steadfast commitment to delivering exceptional care to every resident.


  • Laci is a dedicated healthcare professional with 10 years of experience in the healthcare industry, including three years serving as the Director of Nursing at Family Health & Rehab. She earned her Licensed Practical Nurse (LPN) credential in 2017 and became a Registered Nurse (RN) in 2019. In addition, Laci is certified in Infection Prevention, bringing valuable expertise in resident safety and quality care practices.

    Throughout her career, Laci has remained committed to providing compassionate, resident-centered care while fostering a supportive and collaborative environment for both residents and staff. Her leadership style emphasizes clinical excellence, infection prevention best practices, and a strong commitment to enhancing the overall well-being and quality of life of those she serves.


  • Leslie is an experienced healthcare professional with more than 20 years of experience in the healthcare industry. She began her career working as a Certified Nursing Assistant (CNA) at a local hospital, where she developed a strong foundation in compassionate patient care. Seeking new opportunities to continue growing in her career, Leslie later joined Family Health & Rehab, bringing with her extensive hands-on experience and a deep passion for serving others. She is certified as both a CNA and Registered Medication Aide (RMA).

    Inspired by her grandmother, who worked as a nurse during Leslie’s childhood, Leslie developed an early appreciation for healthcare and caregiving. Her upbringing instilled in her the importance of compassion, empathy, and dedication to others. Throughout her career, Leslie has remained committed to providing quality care and building meaningful connections with residents and their families while supporting a positive and caring environment for both residents and staff.


  • Kyla is a dedicated healthcare professional with 11 years of experience in the healthcare industry, including two years helping lead and manage dietary services teams. She has recently obtained her Certified Dietary Manager and Certified Food Protection Professional (CDM/CFPP) certification, further strengthening her expertise in nutrition, food service management, and resident care.

    Throughout her career, Kyla has been passionate about enhancing the dining experience for residents by creating nutritious, balanced, and enjoyable meals tailored to their preferences and needs. She values resident input and believes in empowering individuals through education, helping them make informed decisions regarding their nutrition and dietary choices. Kyla’s commitment to quality service, resident satisfaction, and compassionate care contributes to a positive and supportive environment for both residents and staff.


  • Danette is a physical therapy professional with 25 years of experience in the field, including 16 years at FHR in skilled nursing and 10 years serving as Director of Rehabilitation.

    Following her college graduation, she dedicated her early career years to raising her family, working only in a per diem capacity until her children reached college age, at which point she returned to full-time clinical practice.

    Danette is passionate about the skilled nursing setting and values the opportunity to work closely with older adults. She finds purpose in serving residents who may feel overlooked or isolated, and she is committed to providing compassionate, patient-centered care. In her leadership role, she prioritizes mentoring therapists and colleagues, fostering a supportive environment through education, active listening, and understanding. She places strong emphasis on professional development and takes pride in helping others grow both in their careers and personal lives.


  • Melodie is a dedicated long-term care professional with more than 17 years of experience in nursing home services and has been a valued member of the Family Health & Rehab team since 2015. Throughout her career, she has gained extensive experience in both dietary and environmental services, contributing to the overall comfort, safety, and well-being of residents and staff.

    In her role within Environmental Services, Melodie works diligently behind the scenes to ensure the facility remains clean, safe, functional, and welcoming each day. Her strong work ethic, attention to detail, and commitment to maintaining a high-quality environment play an essential role in supporting resident care and enhancing the overall operations of the facility.


  • Kari Nusz has built her healthcare career around compassion, leadership, and a deep commitment to patient care. With 7 years of experience in skilled nursing and overall 24 years of long-term care, she has worked in nearly every level of patient support—from CNA and medication aide roles early in her career to leadership positions as a Unit Manager and Assistant Director of Nursing and staffing coordinator  

    Her passion for helping others led her into nursing, where she quickly became known for her ability to stay calm under pressure, think critically, and lead effectively in fast-paced environments. Kari specializes in wound care and infection prevention and is currently expanding her expertise through MDS training.

    Over the years, Kari has gained extensive experience in direct patient care, staffing coordination, scheduling, leadership development, and clinical operations. Whether , mentoring staff, or overseeing daily nursing operations, she brings professionalism, empathy, and attention to detail into every role she serves.

    Kari continues to pursue growth in healthcare leadership while remaining focused on what matters most—providing excellent care and making a meaningful difference in the lives of others.


  • Sandy has worked in healthcare for 11 years. She earned her Certified Nursing Assistant (CNA) certification in 2013, completed her Activity Director certification in 2017, and obtained her Certified Medication Aide (CMA) certification in 2019.

    Sandy brings an enthusiastic, playful, and engaging presence to her Activities Director role. She chose a career in healthcare out of a deep sense of compassion and a strong desire to bring joy to others. Sandy is passionate about building meaningful connections with residents and takes great pride in seeing the positive impact, laughter, and smiles she helps create each day.


  • Sarah has worked in nursing homes for over 20 years, with many of those years dedicated to social services. In recent years, she has also gained experience working in medical records, a role she has greatly enjoyed.

    Sarah finds the most rewarding aspect of working with older adults is helping them feel valued and reminding them that they continue to make a meaningful difference in the lives of others. She takes pride in supporting residents’ emotional well-being and fostering a sense of purpose and dignity. Sarah also finds great satisfaction in organization and attention to detail, which she brings to her work in maintaining accurate and orderly medical records.


  • Claire is the Social Service Director at Family Health & Rehab, where she provides compassionate support and advocacy for residents and their families. With a bachelor's degree in social work from Wichita State University, Claire specializes in case management, care coordination, and connecting individuals with community resources. She is dedicated to promoting dignity, emotional well-being, and quality of life through patient-centered care. Known for their professionalism and empathy, Claire works closely with healthcare teams to create a supportive and positive environment for those they serve.


  • Angel has been in healthcare for over 15 years and is a Licensed Practical Nurse (LPN).

    Throughout her career, Angel has demonstrated a strong commitment to providing compassionate, patient-centered care. She brings extensive clinical experience and a steady, reliable presence to her role, supporting both patients and colleagues in delivering high-quality care. Angel is dedicated to promoting comfort, dignity, and well-being for those she serves and takes pride in contributing to a positive and supportive care environment.


  • Sarah has over 10 years of experience in healthcare and is a Registered Nurse (RN) with an Associate of Applied Science in Nursing degree, which has provided a strong foundation in clinical care and patient advocacy.

    Sarah chose the nursing profession because she has always felt called to care for others and finds fulfillment in helping individuals improve their health and well-being. She finds skilled nursing especially meaningful, as it allows her to empower patients with the education, support, and strength needed to successfully transition back to their home environments.

    In her role as Nurse House Manager at Family Health & Rehab, Sarah has embraced the opportunity to support both patients and staff. She finds great satisfaction in mentoring and guiding employees, helping them grow in their roles while promoting the delivery of high-quality, compassionate care.

  • Nakia has worked in long term care for over 20 years, beginning her career as a Certified Nursing Assistant (CNA). She has served as a Licensed Practical Nurse (LPN) for the past 14 years.

    Throughout her career, Nakia has demonstrated a strong commitment to providing compassionate, quality care to residents in the long term care setting. She brings extensive hands-on experience and a deep understanding of resident needs, consistently supporting comfort, dignity, and well-being. Nakia takes pride in her longevity in the field and her continued dedication to serving residents and contributing to a supportive care environment.


  • Lilly is a Social Service Designee with many years of experience working in elder care.

    She is dedicated to providing comprehensive care coordination and advocacy for residents and their families. Lilly specializes in navigating complex medical needs, coordinating home care services, and developing personalized care plans designed to optimize quality of life. She is committed to ensuring individuals have access to appropriate resources and support systems, with a strong focus on promoting dignity, independence, and overall well-being.
